Central and Eastern Sydney Primary Health Network (CESPHN) takes in the local health districts of Sydney and South Eastern Sydney and the local health networks of St Vincents Hospital and The Sydney Children’s Hospital. The IAR-DST refers to the online Initial Assessment and Referral Decision Support Tool developed by the Department of Health and this tool is also incorporated into the CEPSHN referral form for a number of our commissioned mental health services. The goal of the IAR-DST is to assist general practitioners and other clinicians to determine the most appropriate level of support for a patient/client at a point in time, that is the least intensive intervention likely to result in the best clinical outcome. The tool was developed to ensure a national, standardised, consistent and objective approach to assist with mental healthcare recommendations for individuals accessing mental health care in the primary care sector. The IAR-DST works within the framework of a stepped care approach to mental health referrals and is to be used by a clinician alongside a comprehensive mental health assessment. The Mental Health Services Directory is a local resource for GPs and other mental health referrers working within the Central and Eastern Sydney Primary Health Network (CESPHN). The Mental Health Services Directory was developed to provide a digital resource where relevant local service information can be easily located by GPs and other mental health service providers for patients/clients requiring mental health support. The goal of the directory is to efficiently enable referrers to find services commensurate with the level of support required for their patient/client and as such the directory is designed to be used alongside the IAR-DST (see “What is the IAR-DST?” under main menu), providing easily accessible, up to date information on services mapped to each of the 5 IAR-DST levels of care. To optimise efficiency and the user experience, services can be filtered further by other key variables such as age, postcode, gender, modality of treatment, cost, availability etc to maximise relevant service options.
The intent of this directory is to provide a comprehensive and varied listing of clinical mental health, psychosocial support and crisis services within the CESPHN region, however this list is not exhaustive. CESPHN does not specifically endorse the work of the services listed, but will attempt to ensure all information provided is accurate and regularly updated, so professional judgement can be used to determine whether a service is appropriate for your patient/client.

One Door Mental Health
The Primary Integrated Care Supports provides clinical mental health services to people who are experiencing severe mental illness and live in the Central and Eastern Sydney Primary Health Network area.
This program works to integrate mental health services with general practices; with consumers being allocated a Mental Health Nurse and a Peer Worker to identify their needs and support them in their self-management and recovery. There will be multiple pathways for referral, with the aim of avoiding any unnecessary hospitalisation. Generally, a GP or psychiatrist referral is required.
The PICS mental health nurses may work with peer workers – people with a lived experience of mental illness who have been trained to use this experience to support people in their recovery. Peer workers have a non-clinical role, focusing on supporting people’s needs and recovery. Most often peer workers are supporting people to access the services they need, whether that is sorting out Centrelink payments, getting to a GP appointment or just going for a walk.
Live in the CESPHN region:
Have a severe mental illness and associated associated level of reduced psychosocial functional capacity
Need information or support to access mental health services or NDIS supports
Anyone can refer to or use this service, including: GPs, Allied Health, Community practitioners, carers and consumers.

Experience severe mental illness as evidenced by meeting 3 of the 4 criteria:
Have a diagnosed mental illness which is severe and either episodic or persistent in nature (according to the criteria defined in the World Health Organisation Diagnostic and Management Guidelines for Mental Health Disorders in Primary Care: ICD 10 Chapter V Primary Care Version, or the Diagnostic and Statistical Manual of Mental Health Disorders – Fifth Edition DSM-5)
The mental illness significantly impacts at least two areas of the client’s social, personal and/or occupational functioning
The mental illness has resulted in hospital treatment in the previous two years.
The client is expected to need ongoing treatment and management of their mental illness (for a maximum of two years)
Benefit from accessing primary care mental health services.

The OCD Course is our free online and internet-delivered treatment course designed for people with Obsessive Compulsive Disorder – that is, OCD. The OCD Course is designed to provide good information about OCD and to teach practical, proven, skills for reducing their symptoms and managing their OCD. It was developed because OCD has a significant impact on peoples’ lives but the symptoms often make it very difficult to seek treatment.
People with OCD often also find it difficult to access effective, evidence-based, treatment that works. Online or internet-delivered treatment for OCD is one approach to overcoming some of the barriers people experience and provides access to effective treatment.

The Mental health shared care program provides support to the path of recovery and physical health of a consumer whose care is shared by a GP and the Local Health Networks. Shared Care clearly specifies which service will be responsible for identified aspects of their physical health care.
People experiencing enduring mental illness attend the GP more often but are screened less frequently for common conditions than the general population. As a result, this population have a reduced life expectancy of up to 25 years, with most death due to physical causes.
To be eligible to access the program, clients need to be receiving care coordinated by either SLHD, SESLHD, or St. Vincent’s Health Network Mental Health Services. Contact through SLHD, SESLHD or St. Vincent’s Health Australia Mental Health Services, your GP, or request the service through your care coordinator.

It is a free online and internet-based treatment course designed to help people manage the impacts of neurological conditions on their mental health and day-to-day activities. It was developed because many people with neurological conditions experience significant difficulties with both their emotional wellbeing (e.g., stress, anxiety, low mood) and cognitive wellbeing (e.g., planning, memory, attention). But, many people have limited access to help for managing these difficulties.
The Wellbeing Neuro Course is designed to provide good information and teach practical skills for managing the impact of neurological conditions. However, different to other courses, it is carefully designed for people experiencing cognitive difficulties as a result of having a neurological condition – that is, difficulties with attention, memory, planning, etc.
These courses run 4 or 5 times a year.
The course is suitable for:
Unfortunately, not available for people experiencing very severe cognitive difficulties with day-to-day memory, attention and ability to learn basic information. Or people who are currently suicidal, do not have access to the Internet or who are not living in Australia. Strongly recommended to discuss participation in this Course with their GP and other doctors. Maintaining a good working relationship with your doctors is essential when you have a neurological condition and they can help you decide whether this program is going to be suitable.
